Oxford MPP Ernie Hardeman visited Woodingford Lodge Woodstock this morning to announce financial support to expand Woodingford Lodge Family Transition photo featuring Ernie Hardeman, Jerry Acchione and others Oxford County’s Family Transition Program.  First launched in 2019, the program helps prospective residents and their families adjust to life in long-term care before the day of admission. The funding will allow Woodingford Lodge to invest in the specialized needs of new and existing long-term care residents, help prevent unnecessary hospitalizations, and enable better transitions from hospital to long-term care. An innovative partnership with Home and Community Care Support Services will aid this process, including further collaboration with the Community Paramedicine Program from Oxford County Paramedic Services.

“Funding from Ontario’s Local Priorities Fund will allow Woodingford Lodge to invest in the specialized needs of new and existing long-term care residents, help prevent unnecessary hospitalizations, and enable better transitions from hospital to long-term care. Having launched Canada’s first transition program in 2019 to help prospective residents and their families adjust to long-term care before the day of admission, Woodingford’s Family Transition Program has already helped more than 200 people through this process and, with this funding support, looks forward to helping many more.”
