Ontario’s Local Priorities Fund to support families in transitioning to long-term care
Woodingford Lodge is thrilled to announce that the Family Transition Program has been awarded funding through Ontario’s Local Priorities Fund initiative. This significant support will enable Woodingford Lodge to expand and enhance its services dedicated to improving health outcomes and addressing local health priorities within our community.
The funding will directly support initiatives aimed at helping residents in the community who are awaiting placement into a long-term care home.
The Family Transition Program in Oxford County is designed to provide support for individuals and their families as they make the transition to long-term care. The program aims to ensure a safe and smooth adjustment while also working to decrease the number of emergency visits to the hospital. This funding allows the Family Transition Program to further its mission and better serve the needs of the community.
